This book is a coproduction researched and written by Sacker and Rayne. Both authors have participated in all stages of the word and are fully responsible for the whole text. The writing has primarily been done by Sacker, but always in close cooperation with Rayne. We are very much obliged to Alexis Pavlide for the constructive way she has edited our work.
