Baelor makes the decision to wed Maekar to him after his sons and wife die during the Great Spring Sickness and Baelor assumes the throne. It would be easier if they had spoken since the tournament at Ashford. It would be easier if Maekar had not been in love with him since they were boys.

Summary
Baelor is thirty six, the crown prince, and has buried one wife already. He did not ask for another but a rebellion does not care what you ask for, and so he found himself wed to a brother half his age in the name of Targaryen unity.
Maekar is eighteen, an omega, and has spent three years at Summerhall understanding exactly what he is to his husband; an arrangement. A political necessity. An afterthought, not that this is anything new.
With the rebellion reshaping itself and a unified front to maintain, they are summoned to King's Landing to begin the marriage in earnest.
Neither of them is particularly pleased about it.

His father had the gall to announce that he was to be married, and his opinion did not matter. Again, some pompous lord or his son would have the privilege of pumping their seed into him whenever they pleased.

“Do you think me so cruel that I would make you get rid of the child?” A genuine look of hurt flashes across his face, his blue eyes studying him intently.
Perhaps he was wrong.
“No, Robert.” He feels a surge of hope. Perhaps he will show Jon the mercy he denied his siblings, now that he has calmed down. “I thought it best not to tempt fate.” He grabs him by the forearm. “I can still take him to the North. Nobody has to know the truth. He won’t either. He will be no threat to you or your rule. I will make sure of that.”
Robert chuckles. “It was already too late for that the moment you crossed the city gates. Before nightfall, every beggar and whore will know the truth, or some twisted version of it.” He glances towards the cradle. “Jon. I don’t like this name. My son should have a proper Baratheon name, but since you took it upon yourself to name him as such, I have no choice but to accept that.”
Ned freezes. “Your son?”
