Summary
John is a mentally unstable patient at Vought Ward who is known for experiencing hallucinations in which he believes he is a superhero called The Homelander. He is completely convinced that his delusions are real and often draws other patients into them. In his mind, the substance-abusing patients become "A-Train" and "Queen Maeve," while doctors are reimagined as Hughie, annie ''Starlight'', and numerous other figures from his imagined world. He frequently speaks about having a son and views Dr. Billy Butcher as his nemesis.
Billy is a doctor at Vought-Ward who appears to take pleasure in tormenting John. Rather than challenging his patient's delusions, he often encourages them, playing along with the fantasy and embracing the role of Homelander's nemesis.

Summary
It's all Ryan's fault that Butcher's stuck with this pathetic little piece of shite, now that Homelander's lost his powers. Just because the soft wee lad can't bear to see one dad kill another. Something something trauma. As if this whole family isn't any therapist's wet dream - the rational part of his brain knows they should all go to therapy, whatever good that's gonna do. But he's only go so much time left. And he's only promised Ryan to keep that snivelling twat alive, not that he wouldn't make him suffer.

All the warnings, honestly. I just want them all to suffer, Homelander most of all. And the finale gave me ideas :)

Summary
The long-waited opportunity had arrived. All that he ever wanted, right before his eyes -- Homelander beaten, powerless, bleeding at his feet, and for the first time in his life, afraid. Begging. Desperate.
All Billy had to do was finish it.
Yet, less than an hour later, he found himself on the road, on a one-way trip to a certain safehouse with a broken Homelander bleeding into those shabby car seats. Initially telling himself it's only temporary, postponing an execution he swears is still coming.
But as hatred gives way to habit, and habit to something far more dangerous, Billy finds himself haunted by a question he cannot answer:
If he truly intended to kill John, why did he save him in the first place?

It all collapses onto Butcher, a single moment of pain, the sort of empathy only his brother was capable of. He can’t merge the monster with the child in front of him. Like he’s suddenly wearing red boots five sizes too large in a costume that’ll never fit, confused at why this big, scary man is hurting him.
Or; Butcher tries to kill Homelander, but only sees a child—and what was done to him.
