Summary
Astarion should have died that night. In the story we know he did exactly that. Here, things went a little differently and he is very much alive.
However, healing magic has its limitations, and the Astarion who survived is markedly different from the one that fell to the Gur's clubs.
A month into recovery, he's still coming to terms with his loss of independence. Bedbound, partially mute, and being cared for by the same person who saved him in the first place: his mother. Apparently her adventuring days didn't fully end, even after decades of playing at being a proper Upper City wife.
Even worse, soon his beloved father decides caring for Astarion is a waste of time and resources, and tries to convince his mother to send him to... somewhere, anywhere, else.
It's amazing, what you start to notice when you're forced to stop and pay attention to people not named Astarion. So many aspects of his world and family, he now sees all too clearly. Out of the one eye that still works, anyway.

Summary
Back at it again at the Krispie Kreme, Avernus version.
Karlach is trapped here, fighting every day in this horrible literal hell pit. But this time, she isn’t alone! Astarion and Wyll are here! But… gods, maybe that is making this shit worse, instead of better? Astarion keeps getting injured in battles, and Karlach doesn't feel any less guilty about things when he repeatedly says he's fine, he was tortured all the time before, it’s nothing to him, look how fast he heals.
Karlach was already scared she had brought him to Avernus only for him to die, instead of getting to enjoy his new free life, then today he managed to fall into a pit of lava during a battle. They fish him out, but now Karlach has to deal with protecting a badly burnt and feetless vampire on top of all of the other horrors of the hells.
And he won’t stop making jokes about the situation long enough for her to fucking think! ARGH!!!

Astarion gets horrifically injured but is more worried about making sure Karlach isn’t sad about it.

Summary
Astarion rarely rests well, and this evening is no exception. Horrible visions when he trances, followed by a terrible nightmare. Which is actually Karlach’s nightmare, shared with him due to their little wiggling friends. She’s refused to tell him anything about her time in the hells, despite how much he's asked. Time for him to get the answers he thought he wanted, in glorious Tadpole-o-vision.
After they wake, she tells him she had a very different dream, nothing to worry about. What’s a poor vampire to do? He's supposed to be the liar, not her!

TL;DR - hey lets look at the BG3 story and expand out how Karlach and Astarion mighta become a couple.
Astarion and Karlach, at first glance these two companions couldn't be more different.
She's so open and loud and friendly and eager to do good. He's nothing like any of that.
But turns out, they have way more in common than either would have suspected.
Maybe a slight, sullen, suspicious vampire could use somebody who understands, but is nothing like him.
Even better if it turns out maybe she is a little bit like him.

Summary

There is a bit of that someone left in the orb lodged in Gale's chest.

The woman tilted her head. "Do you know what we just did to you?"
Astarion's eyes darted between them, perplexed by the question. What they did to him…?
That's when he noticed the Selûnite pendant around her neck. She was a cleric… had she healed him? He tried to piece together a scenario that made any sort of sense, but the details barely added up. Had these people found him? If so, why did they drag him across the city? How did they do such a thing? Why wasn't he in Harborside getting treatment? He wasn't sure exactly how severe his injuries had been, but he knew for a fact that a simple healing potion would not have done a thing to mend him back together.
"Are you… healers?" he asked.
The bearded man's mouth dropped open. "Healers—? What? No," he began to reach for him, but thought better of it and awkwardly closed his fist instead. "Astarion, it's me. Gale."
Astarion didn't recognize his name or his face in the slightest, but the way he was looking at him gave him pause.
"I… I don't…" Astarion shook his head slightly, racking his brain. "I'm sorry… Have we met before?"

Summary
The old lady struggles to keep her eyes open. Her bleary gaze slides over towards Halsin and in the next moment her bloodless lips stretch in a wide smile of recognition.
Astarion takes a step forward. That gets the lady's attention; her eyes widen just a little as she lets go of Halsin's hand and stretches out hers towards Astarion.
"Jimmy… you came…"
"Of course I came, my beautiful love. Of course I came," Astarion says softly. He reaches for her, his own hand radiating barely visible black rays. Halsin's eyes are glued to his fingers, following their every move. Astarion gives him a quick sideways glance. Halsin knows to resist the allure of the black aura. Still, Astarion checks, just to be sure.
Something ends, something begins.
In a world defined by forces most people aren't even aware of, Halsin and Astarion navigate their work for a mysterious Agency, coping with the consequences of their interference with mortal lives, and making their relationship work despite their differences.
