Summary:

Law is a stressed surgeon, meanwhile Luffy is a chaotic college student/firefighter who breaks his knee. The slow burn takes abit as Luffy keeps ending up in the hospital because of his injuries

Chapter 1: Night Shift
The emergency room at Grand Line General Hospital smelled like antiseptic, burnt coffee, and the faint metallic tang of blood that never quite washed out of the air. Dr. Trafalgar Law rubbed his temples, staring at the chart on the screen in front of him. Another twelve-hour shift bleeding into fourteen. His white coat hung open over dark scrubs, stethoscope draped around his neck like a noose he’d grown too used to.
“Dr. Law, you’ve got another one,” a nurse called from the bay. “Trauma from a party stunt gone wrong. Knee looks messed up.”
Law exhaled through his nose. “Room 4?”
“Yep. Kid’s loud. Brazilian, twenty, firefighter on the side. Name’s Monkey D. Luffy.”
Law didn’t react. Names blurred together after years in the ER. He grabbed his tablet and walked down the hall, shoes squeaking softly on the linoleum. Bepo—tall, pale-skinned with his shock of white hair and gentle eyes—fell into step beside him, chart in hand.
“Shachi and Penguin are handling the minor stuff tonight,” Bepo said quietly. “This one’s yours. Guy apparently jumped off a balcony trying to ‘fly’ with some rope contraption his friends built. Landed wrong.”
Law’s lip twitched in what might have been annoyance or exhaustion. “College kids.”
He pushed open the door to Room 4.
The patient was sitting up on the gurney despite the obvious pain, one leg extended and already braced by paramedics. Messy black hair, bright grin even through the wince, and a tank top that had seen better days. Short—maybe 5’8—compact and wiry like someone who ran headfirst into life without brakes. A tall Japanese guy (about 5’11, green hair, swordsman vibes) stood next to him with arms crossed, looking more irritated than worried.
“—and then Zoro said it wouldn’t hold, but I bet him it would! Shishishi, it was awesome until the end,” the kid was saying, voice carrying like he was at a barbecue instead of the ER at 2 a.m.
Law stepped in. “Monkey D. Luffy?”
The grin turned toward him. Wide, sunny, completely unbothered. “Yeah! That’s me. You the doctor? You’re tall.”
Law ignored the comment. “I’m Dr. Trafalgar. Let’s see the knee.”
He moved efficiently, gloved hands gentle but firm as he examined the joint. Swollen, already bruising badly. Possible fracture or torn ligaments. The kid hissed once but didn’t pull away. Up close, Luffy smelled like smoke, cheap beer, and something like sea salt—odd for someone who’d just fallen off a balcony.
Zoro grunted. “He’s an idiot. I told him not to.”
Luffy laughed. “It was fun though! Sanji’s probably still yelling back at the dorm.”
Law kept his eyes on the imaging request he was typing. “We’ll need X-rays. Possible patellar fracture or ACL damage. You’re a firefighter?”
“Part-time! With Zoro. Mostly I’m in college. Marine biology, but I skip a lot.” Another bright laugh, like pain was just background noise.
Law didn’t smile back. He was German, built like he played basketball in another life—6’3 and lean from too many skipped meals and too much caffeine. At twenty-seven he felt ten years older some nights. This twenty-year-old ball of chaos was the exact opposite of the controlled environment he preferred.
“Stay still for the techs,” Law said flatly, already turning toward the door. “No more flying experiments.”
He caught the kid’s eyes for half a second—dark, curious, and far too interested. Luffy tilted his head. “Your accent’s cool. Where—”
But Law was already gone, door swinging shut behind him.
Out in the hall, Bepo raised an eyebrow. “He seems… energetic.”
“Idiot,” Law muttered, rubbing the back of his neck. “Page me when the films are back.”
He headed for the break room, already thinking about the triple espresso he’d need before the next trauma rolled in. Somewhere behind him, he could still hear muffled laughter from Room 4.
This was going to be a long night.
