John caught his reflection in the window of the coffee shop as he walked by, he stopped for a moment almost not recognize the man staring back at him. He had lost more weight than he thought and his beard was now long and unkempt same as his hair. What if Sherlock could see him now, would he even know who he was. But then that didn't matter, he chastised himself before he continued walking, Sherlock was dead and he was not coming back.
John rounded the corner and headed for his hiding spot far from the prying eyes of the CCTV cameras that still seemed to follow him each day. He had gotten good at staying out of sight from them. You would think Mycroft had better things to do than to watch his dead brother's friend wonder the streets and fall apart.

Summary
After Sherlock overdoses on some strange mixture for a case, his Mind Palace leads him to situations he wasn’t ever prepared to deal with. His subconscious reveals he has more sentiment than he thinks: sentiment particularly with his infatuation over John. Not only that, but he’s stuck in the flat for four weeks of post-near-death recovery, and both he and John are figuring out how to do that.
Moreover, Sherlock has trouble accepting a part of him craves something so pedestrian.

I truly love when a writer addresses the struggles a person faces when someone they care for takes their life. In this instance the struggle to move past it having happened in front of you.
John is a strong character but he holds so much in. Sherlock needed to realize that his reasons for being abroad and facing unknown evil were honorable but hardly without consequences.

“You know, Donovan,” a deep voice said, interrupting her thoughts. The detective lifted a few papers from the small table beside Hamish’s bed before continuing. “I don’t care if you insult me or even if you question my relationship with John. Honestly, after all this time I've heard all your insults and I expect nothing more from you.” By this time he was holding the case files, which had been next to one of Hamish’s bedtime stories.
He gave her a cold glare. “But refrain from saying such idiocies in front of my own son. Not for John’s sake, or even for mine—especially not for mine,” he said the latter with a bit of a scoff. “But for Hamish’s. He’s too young to be subjected to bullies and their hurtful comments.”
Donovan was somewhat shocked silence.
She stayed motionless as the detective made his way past her and down the stairs without another word.
